Third Party Service Provider Permission

In 2021 Department of Education created a Policy to protect information shared with third party services. This new process requires guardians to consent for apps and websites we use which collect student information.

Most families were sent a link via Flexischools on Wednesday to complete the consent ‘form’ online. We have attached it here for those parents who do not use Flexischools or have not yet completed the form. Each student needs a separate consent form to be completed before they will be able to access any devices at school. We ask that forms are completed by COB Thursday 18 February 2021.

As a school we have added all apps and websites used across the school and permission is for your child’s entire schooling at Mount Hawthorn Primary School unless you withdraw it in writing. As the Department continues to assess the Third Party Services available you may be asked to complete further consent forms throughout the year.

The form has four parts to complete:

  • ​The first section of the form is the online service account permission, this form is completed at enrolment but as we are updating our information we are asking parents to complete it.
  • The notification section are apps and websites that are used at the school that the Department has classified as the school requiring to notify parents that they are being used, these are very low risk.
  • The bundled consent section, has a list of all websites and apps across the school that the Department has rated as minor risks identified with this service. At the end of the list there is a consent section to sign for all apps and websites in the bundled consent category.
  • The explicit consent section, has a list of all websites and apps across the school that the Department has rated as some security and / or privacy risks identified with this service. At the end of the list there is a consent section to sign for all apps and websites in the bundled consent category.

ACCEPTABLE USE AGREEMENT
Also, on Monday your child will bring home the Acceptable Use Agreement form, this was revised last year with consultation from the school board resources sub-committee. Please read through with your child, sign and return to school ASAP.
